    I would point to driver error I'm most cases, BUT, there are time when engineering isn't doing any one any favors. Having not driven the truck Brokenwing speaks of I take his word that there is an engineering flaw that increases the likelihood, perhaps substantially that an error could be made. An untimely acceleration on a spot of sand or black ice could catch even the most experienced driver off guard. Now could you still blame that on the driver? Sure, but proper design would decrease the chances dramatically. And would believe that the manufacturer would not care enough until the bottom line swings the wrong way.
