I believe you have the fliberty gibet confused with the widget/doodad assembly. No big deal, as it's a common mistake.
Excessive strain on the fliberty gibet is the usual cause.
40:1 is what I run in all of my saws, Old and New.......tuned properly, it is cheap insurance in preventing damage.
