Harman XXV TC fire door handle fell off! C-lock washer for the fix?

    The bolts on the inside of the fire door that hold the door handle latch got very loose and with just a pull on the handle it fell right off!
    Adding 2 c-lock washers under the bolts should keep them from getting loose again.
    Has anyone else seen this?
